I was sent out a Ameriquest Multistate settlement Release Form, which I signed, and filled out all the requested information, and returned it in the envelope that came with it, going to (Ameriquest Settlement Administration, PO Box 1855 Faribault, MN 55021-7110).
A few weeks later, I received a second notice requesting that I do the same, that they have not received it. The envelope had to be postmarked by September 10, 2007, I sent it out a second time on August 15, 2007 from Poughkeepsie, NY I also made a copy of the form this time. I recently called 1800-420-5875 to find out when my check was going to be sent out, and I was told that they didn't receive the form. I find it hard to believe that two out of two times the post office has lost my mail going to the same place on seperate mailing days.
